Phase I ESA: Interview Questionnaire - Owner/Manager

As part of the development of an Environmental Site Assessment, our team is responsible for collecting as much information as possible regarding the historical and current use of the property and any known environmental concerns. This questionnaire serves to collect the required information to assist our client (the purchaser) meet their requirements to complete the purchase of the property. Please complete the below questionnaire to the best of your ability. The more information you can provide is of benefit.

Please identify any changes to the use of the property you have made during the current ownership.

Please provide any of contacts (names and email addresses) of individual who may know more information regarding the current use or history of the property with respect to this environmental assessment. (This can include a site manager or individual who is knowledgeable of the daily operations of the property)

Please describe the current use, storage, and disposal for any hazardous substances and any regulation or required processes by regulators.

Please describe any knowledge you have of the past use of the property and any environmental concerns prior to, during, or after you purchased the property, with respect to the use by previous owners/tenants.

As the current owner of the property, please describe any environmental concerns with adjoining (neighboring) properties or properties in the surrounding area you are aware of.
